Job description
**Job Title: Certified Nursing Assistant (CNA)**
The CNA is responsible for delivering nursing procedures and patient care as specified in the individualized care plan for each patient. They are tasked with observing and reporting any changes in the patient’s condition to the RN Case Manager, as well as ensuring that thorough documentation of the care provided is maintained. Applicants should possess a current and valid Wisconsin Nursing Aid certification. A high school diploma is preferred, and prior experience in hospice or palliative care is strongly recommended.
