Job Summary
We are seeking a dedicated hospice nurse to provide personalized nursing care in the homes or facilities of patients. This role encompasses a variety of responsibilities, including the assessment of patient needs, developing care plans, administering medications, managing symptoms, and maintaining precise documentation. A collaborative approach with an interdisciplinary team is essential, alongside the education and support of both patients and their families.
Responsibilities
- Deliver individualized hospice nursing services to patients in their homes or care settings.
- Conduct thorough assessments and create tailored care plans for patients.
- Administer medications effectively while managing symptoms and their alleviation.
- Maintain accurate and complete documentation of patient care and progress.
- Work collaboratively with the interdisciplinary team to ensure comprehensive patient care.
- Provide education and emotional support to patients and their families throughout the care process.
- Participate in case conferences and engage in ongoing professional development activities.
Qualifications
- A current and valid Michigan Registered Nurse license is required.
- Minimum of two years of nursing experience in relevant settings.
- Current CPR certification is mandatory.
- Experience in hospice, palliative care, or home health is preferred.
- Solid understanding of end-of-life care and symptom management is essential.
- Strong communication and teamwork abilities are crucial for effective collaboration.
- Must demonstrate independence and a compassionate, patient-focused approach in all interactions.
