Job Summary
The RN Case Manager plays a crucial role in providing initial assessments of patients and crafting detailed care plans that cater to the requirements of patients and their families. This position focuses on administering assigned treatments, overseeing patient conditions, and managing discharge processes effectively.
Responsibilities
- Conduct initial assessments of patients upon admission.
- Develop and implement comprehensive care plans tailored to individual patient needs.
- Administer prescribed treatments as directed.
- Monitor patient progress and adjust care plans as necessary.
- Coordinate the discharge planning process to ensure a smooth transition for patients.
Qualifications
- Current and valid RN license from the state.
- Minimum of one year of recent experience in an acute care setting.
- Valid driver’s license and auto insurance.
- Certification in Basic Life Support (BLS).
- Bachelor’s degree in nursing is preferred.
