Job Summary
The Hospital Liaison serves as a vital link between healthcare facilities and the organization, ensuring smooth communication and collaboration. This role focuses on fostering relationships with hospitals to enhance patient care and support the transition process for patients needing continued services.
Responsibilities
- Establish and maintain effective partnerships with hospital staff to promote agency services.
- Facilitate smooth patient transitions from hospital settings into follow-up care.
- Act as a key point of contact for inquiries and updates regarding patient status and needs.
- Collaborate with interdisciplinary teams to ensure appropriate resources are provided to patients.
- Participate in community outreach programs to enhance awareness of available services.
Qualifications
- Bachelor’s degree in a relevant field such as healthcare administration, social work, or nursing preferred.
- Demonstrated experience in a healthcare setting, particularly in liaison roles or case management.
- Strong communication and interpersonal skills are essential for relationship building.
- Ability to work independently and manage multiple priorities effectively.
- Knowledge of patient care processes and healthcare regulations is a plus.
