**Job Title: Intake Coordinator**
The Intake Coordinator is responsible for overseeing the enrollment process for patients by providing information to prospective clients and collaborating with clinical teams and referral sources. This role includes ensuring that documentation is accurate and assisting in the initiation of care for new clients. Applicants should hold a Bachelor’s degree and have 3-5 years of experience within a home care environment, along with a solid understanding of New York State Department of Health operations. Effective management of a high volume of intake responsibilities necessitates strong organizational, communication, and technical abilities.
