**Job Title: Youth Care Coordinator**
In this role, you will implement care planning and coordinate services for youth through the Wraparound approach. You will lead care planning team meetings and support families in navigating various systems, including educational, judicial, and state agencies. A Bachelor’s degree in Social Work, Psychology, or a similar field is required. Applicants should have a minimum of one year of direct experience with children, adolescents, or families and must hold a valid driver’s license.
