Job description
Title: Social Worker (LCSW)
Provide assessment, guidance, and support for patients with complex medical conditions by creating tailored treatment strategies. Aid in adapting to disabilities, connecting to necessary resources, and implementing behavior modifications within the patient’s home environment and community settings. A Master’s degree in Social Work and state licensure as an LCSW is required. Strong abilities in planning, organization, writing, and time management are essential for candidates.
