Job description
In this role, you will evaluate, advise, and guide patients facing medical complexities by creating tailored treatment plans. You will support their adaptation to disability, connect them with resources, and apply behavior modification techniques. Applicants should hold a Master’s degree in Social Work and possess state licensure as an LCSW. Strong skills in planning, organization, written communication, and interpersonal communication are essential for success in this position.
