The LPN will also be tasked with observing client health statuses, facilitating care coordination, and ensuring precise documentation through electronic medical records. Applicants should possess an active Pennsylvania LPN license and have a minimum of one year of recent, confirmed clinical experience. Excellent communication abilities, proficiency with computers, and a willingness to travel to clients’ residences are essential.
