Job description
The Licensed Practical Nurse (LPN) will carry out home visits to deliver skilled nursing services, which encompass medication management, wound treatment, and patient evaluations. The LPN will also be tasked with observing client health statuses, facilitating care coordination, and ensuring precise documentation through electronic medical records. Applicants should possess an active Pennsylvania LPN license and have a minimum of one year of recent, confirmed clinical experience. Excellent communication abilities, proficiency with computers, and a willingness to travel to clients’ residences are essential.
