Job description
The Licensed Practical Nurse (LPN) will oversee personalized health care services for individuals with disabilities, which includes performing site assessments, monitoring vital signs, and conducting medication reviews. Additionally, the LPN will serve as a medical resource for the team and ensure adherence to both organizational and state medical regulations. Applicants must possess an active LPN license in the State of Indiana. Candidates with 1-2 years of experience in working with individuals with disabilities or in long-term care environments will be favored.
