Provide direct care to assisted living or memory care residents, administer medications, conduct assessments, monitor changes in condition, and document care. Maintain individualized care plans, participate in interdisciplinary meetings, monitor vital signs and other measures, and support and train direct care staff while promoting fall prevention. Requires a high school diploma or GED, an active Licensed Practical Nurse license in the applicable state, current CPR certification, and at least two years of related experience. Candidates should know medical terminology, medical records management, and clinical or diagnostic procedures, and be able to meet the role’s physical requirements.
