Job Summary
The Licensed Practical Nurse (LPN) will engage in the organization and delivery of patient care both at home and within community environments, adhering to predefined nursing care protocols. This role involves effective communication regarding patient health statuses and supporting individuals with their daily living tasks while ensuring a secure atmosphere for those under their care.
Responsibilities
- Participate in the planning and organization of patient care in various settings.
- Follow established nursing care plans to facilitate the best patient outcomes.
- Accurately report on patient conditions to relevant healthcare professionals.
- Assist patients with daily living activities to enhance their quality of life.
- Maintain a safe and hygienic environment for patients to ensure their well-being.
Qualifications
- Minimum of one year of experience as a Licensed Practical Nurse in a home care environment.
- Valid state nursing license.
- Current CPR certification.
- Current TB test certification.
- Possession of a valid driver’s license and reliable transportation.
