Job Summary
The role involves delivering both individual and group therapy sessions for residents in skilled nursing facilities, focusing on the creation and implementation of evidence-based treatment plans. This position requires close collaboration with interdisciplinary teams and facility personnel to ensure effective monitoring of patient progress while maintaining thorough clinical documentation.
Responsibilities
- Conduct individual therapy sessions and group psychotherapy for residents.

- Monitor and document patient progress accurately to ensure continuity of care.
- Provide in-person therapy services at the assigned skilled nursing facilities.
- Utilize electronic health records for documentation and record-keeping purposes.
Qualifications
- Candidates must possess an active and unrestricted license as a Licensed Professional Counselor in the state of practice.
- Demonstrated proficiency with electronic health records is essential.
- Ability to provide in-person services at designated facilities is required.
