Job Summary
The Long Term Care Options Counselor provides expert guidance and support to individuals and families navigating long-term care choices. This role involves assessing needs, offering information on available services, and facilitating access to resources that promote informed decision-making.
Responsibilities
- Conduct assessments to determine the long-term care needs of clients.
- Provide detailed information regarding various long-term care options.
- Assist clients in understanding and accessing available resources and services.
- Support clients and their families in making informed care decisions.

Qualifications
