**Job Title: Long Term Care Options Counselor**
The Long Term Care Options Counselor is responsible for conducting thorough needs assessments and creating individualized care plans tailored to the requirements of individuals in need of support. This role involves providing counseling across different environments, such as private residences and care facilities, and contributing to public education initiatives. Applicants should possess a Bachelor’s degree in a human services discipline or an Associate’s degree paired with pertinent experience. Prior experience with elderly individuals or adults with disabilities is essential, as well as holding a valid driver’s license and having dependable transportation.
