Job Summary
The Licensed Practical Nurse (LPN) in a private duty setting is responsible for delivering personalized nursing care to patients in their homes. They play a crucial role in monitoring health conditions, administering medications, and providing support to patients and their families, ensuring quality health care delivery tailored to individual needs.
Responsibilities
- Assess and monitor patients’ health statuses and vital signs.
- Administer prescribed medications and treatments according to established protocols.
- Assist with personal hygiene and daily living activities for clients.
- Communicate effectively with patients, families, and healthcare team members.
- Maintain accurate patient records and documentation of care provided.
- Participate in developing care plans and ensure adherence to them.
Qualifications
- Current and valid LPN license in the state of Indiana.
- Previous experience in a home health or private duty nursing setting is preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and deliver high-quality patient care.
