**Job Title: Case Manager**
The Case Manager will perform psychosocial evaluations and create tailored service plans aimed at assisting clients with HIV in navigating obstacles to their care. This role involves overseeing an active client caseload, guaranteeing that individuals receive essential medical and support services while meticulously maintaining documentation that aligns with program guidelines. Applicants should have a bachelor’s degree in social work or a related discipline, or possess a minimum of four years’ experience in relevant case management. A solid grasp of Microsoft Office, excellent communication abilities, and a thorough knowledge of HIV-related resources as well as cultural sensitivity are essential for this position.
