Job Summary
The Licensed Vocational Nurse (LVN) in Home Health is tasked with delivering essential nursing care to patients in their residence. This role is crucial in ensuring that patients receive continuous health monitoring, assist with daily living activities, and contribute to recovery processes using effective nursing practices.
Responsibilities
- Administer medications and treatments as prescribed by healthcare professionals.
- Monitor patients’ condition by observing vital signs and reporting any changes.
- Assist patients with personal hygiene and mobility.
- Document patient care activities and maintain accurate records.
- Communicate effectively with patients, families, and the healthcare team to ensure quality care.
- Educate patients and families on health management and care procedures.
Qualifications
- Valid LVN license in the state of practice.
- Previous experience in home health or a similar healthcare setting.
- Strong communication and interpersonal skills.
