Job Summary

The LVN Sub Acute Per-Diem Days position is designed for a compassionate and skilled Licensed Vocational Nurse to provide high-quality care in a sub-acute setting. This role focuses on delivering essential medical and personal support to patients, ensuring their comfort and safety during their rehabilitation process.

Responsibilities

  • Deliver nursing care to patients in a sub-acute environment, tailoring services to individual needs.
  • Assist with daily living activities and ensure a safe, comfortable environment.
  • Administer medications and treatments as prescribed by healthcare professionals.
  • Monitor patient conditions and report significant changes to the healthcare team.
  • Educate patients and their families regarding care plans and health improvements.
  • Collaborate with other healthcare staff to provide holistic care.

Qualifications

  • Current and valid LVN license in the state.
  • Experience in a sub-acute or similar healthcare environment preferred.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic knowledge of patient care protocols and emergency procedures.
