Job description
**Job Title: Nurse Care Manager**
The Nurse Care Manager is responsible for organizing and prioritizing client care, which includes performing home visits to evaluate individual needs and create tailored care plans. This role involves working closely with healthcare professionals and community organizations to maintain seamless care and enhance patient health results. Applicants should have an Associate’s degree in nursing, social work, or a related health discipline, along with a minimum of 2 years of pertinent experience. A current Registered Nurse (RN) license in Michigan and Basic Life Support (BLS) certification are mandatory qualifications.
