Job Summary
The MDS Coordinator plays a vital role in overseeing patient care plans and facilitating coordination among various healthcare professionals. This position ensures meticulous documentation of medical records while effectively communicating treatment updates to patients’ families. Furthermore, the MDS Coordinator is responsible for implementing physician directives concerning medication and care protocols.
Responsibilities
- Oversee and manage patient care plans.
- Coordinate care activities across different healthcare disciplines.
- Maintain accurate and comprehensive medical records documentation.
- Communicate progress of treatment to families and caregivers.
- Uphold and implement physician orders related to patient medication and care.
Qualifications
- Must hold a valid license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in Indiana.
- Demonstrated ability to manage complex patient medical histories.
- Experience coordinating care across multiple healthcare facilities.
