The Medical Social Worker will conduct psychosocial assessments and provide counseling to patients and families in their homes. They will also coordinate with clinical staff to connect patients with necessary community, financial, and long-term care resources. Candidates must hold a Master’s Degree in Social Work (MSW) and possess a full, active, and unrestricted social work license. Additionally, applicants are required to have a valid driver’s license, a reliable personal vehicle, and current auto insurance.
