Job description
**Nurse Practitioner**
The Nurse Practitioner is responsible for delivering direct patient care, which encompasses geriatric evaluations, diagnosing conditions, and developing treatment plans in diverse clinical and home environments. This role involves working closely with a multidisciplinary team to oversee patient management, aid in care transitions, and maintain adherence to quality and regulatory guidelines. Applicants should possess an ARNP or an equivalent advanced nursing qualification, a valid Illinois license, and full practice authority. A background of 3-5 years in managing a primary care patient panel is essential, as well as certification in Basic Life Support (BLS).
