Job Summary
The role involves providing extensive primary healthcare services and conducting physical assessments directly within patients’ residences to effectively manage long-term health issues. Responsibilities
- Conduct thorough primary care and physical examinations in patients’ homes.
- Manage patient care related to chronic health conditions.
- Collaborate with a multidisciplinary team to ensure effective care transitions.
- Work proactively to avoid unnecessary hospitalizations.
Qualifications
- Master’s degree as a Nurse Practitioner with a specialization in Adult, Geriatric, Acute Care, or Family practice.
- Possess an active and unencumbered Pennsylvania state nursing license.
- Hold board certification in the relevant specialty.
- Valid driver’s license to facilitate travel for home visits.
