Job Summary
We are seeking a Nurse Practitioner or Physician Assistant to join our team on a PRN basis, focusing on conducting in-home health assessments. The position is located in Meherrin, Virginia, with a requirement for onsite work. Matrix Medical Network, a pioneer in the field, leads the way in providing comprehensive healthcare including diagnosis, treatment, health education, and case management referrals directly in patient residences or skilled nursing facilities.
Responsibilities
- Conduct in-home health assessments for patients.
- Perform diagnosis and treatment based on assessment findings.
- Provide health education and information to patients and families.
- Facilitate physician referrals as needed.
- Engage in case management referrals to ensure comprehensive patient care.
Qualifications
- Must be a licensed Nurse Practitioner or Physician Assistant.
- Experience in home health assessments or similar settings is preferred.
- Strong communication and patient assessment skills.
- Ability to work independently and demonstrate initiative in patient care.
