Job Summary
This position involves performing thorough in-home health assessments for elderly and at-risk individuals, focusing on identifying chronic health issues and enhancing overall care outcomes. Providers in this role are expected to work closely with primary care physicians, caregivers, and other healthcare professionals to guarantee proper follow-up and maintain accurate documentation.
Responsibilities
- Conduct in-depth health assessments for older adults and individuals at risk.
- Identify chronic conditions and recommend necessary interventions.
- Collaborate with primary care physicians and other healthcare staff to coordinate care.
- Ensure detailed documentation and effective follow-up on health assessments.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner licensure with prescriptive authority.
- Board certification in a pertinent specialty.
- Valid certification in BLS, ACLS, or CPR.
