Job Summary
The role involves performing detailed in-home health evaluations for members, focusing on assessing medical histories, monitoring medication compliance, and identifying social determinants that may impact health. Responsibilities
- Conduct thorough in-home assessments of health status and needs.
- Review and evaluate members’ medical histories and medication adherence.
- Identify and assess social risks that could affect health outcomes.
- Work in partnership with primary care physicians to develop care plans.
- Collaborate with caregivers to ensure effective health management.
- Support strategies aimed at closing care gaps and improving overall health outcomes.
Qualifications
- Master’s degree in a related field.
- Current Nurse Practitioner licensure with authority to prescribe medications.
- Board certification in a pertinent specialty.
- Current certifications in BLS, ACLS, and CPR.
