Job Summary
This position involves performing detailed in-home health evaluations for adult and elderly patients, focusing on their medical histories, chronic illness identification, and addressing care gaps. The role emphasizes collaboration with primary care doctors, caregivers, and medical teams to facilitate coordinated care and maintain precise documentation.
Responsibilities
- Conduct thorough in-home health assessments for adult and geriatric members.
- Review and analyze medical histories to identify chronic conditions.
- Work in partnership with primary care physicians and caregivers to ensure seamless care coordination.
- Document findings accurately and comprehensively.
- Close identified care gaps for improved patient outcomes.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner licensure with authorization to prescribe medications.
- Board certification in a pertinent specialty.
- Current certification in BLS, ACLS, or CPR.
