Job Summary
This role involves conducting thorough in-home health evaluations for elderly and high-risk individuals, with a focus on understanding their medical history and diagnosing chronic illnesses. The goal is to work closely with primary care physicians, caregivers, and ancillary staff to enhance the quality of care and address any existing care gaps.
Responsibilities
- Perform detailed health assessments for older adults and at-risk populations.
- Examine medical histories to identify and document chronic conditions.
- Collaborate with healthcare professionals including primary care physicians and caregivers.
- Ensure effective communication and coordination to provide quality care.
- Identify and address care gaps to improve patient outcomes.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner licensure with prescriptive authority.
- Board certification in a related specialty area.
- Up-to-date BLS, ACLS, or CPR certification.
