Job Summary
The role involves performing thorough in-home health evaluations for elderly and at-risk populations to pinpoint chronic illnesses and enhance overall wellness. Providers will work alongside primary care doctors, caregivers, and other medical staff to provide cohesive care and maintain accurate documentation of patient health status.
Responsibilities
- Conduct detailed health assessments in home settings for older adults and individuals at risk.
- Identify chronic health conditions to facilitate improved health outcomes.
- Collaborate effectively with primary care physicians and medical teams.
- Ensure meticulous documentation of health information and treatment plans.
Qualifications
- A Master’s degree in a relevant field.
- Valid Nurse Practitioner licensure with authority to prescribe medications.
- Board certification in a pertinent specialty area.
- Possess current certifications in BLS, ACLS, or CPR.
