Job Summary
The Nurse Practitioner will perform thorough in-home health evaluations for senior citizens and individuals considered at high risk, which will encompass the assessment of medical histories as well as diagnostic examinations. The role demands interaction and teamwork with primary care doctors, case managers, and caregivers aimed at enhancing patient health results and addressing any gaps in care.
Responsibilities
- Conduct detailed assessments of the health status of older and at-risk individuals.
- Review medical histories and perform necessary diagnostic tests.
- Collaborate effectively with healthcare professionals to optimize patient care.
- Identify and address care gaps to improve patient outcomes.
Qualifications
- A Master’s degree in a relevant field.
- Current Nurse Practitioner license with authority to prescribe medication.
- Board certification in an appropriate specialty.
- Valid BLS, ACLS, or CPR certification.
