Job Summary
This role involves performing detailed in-home health evaluations for seniors and individuals at risk, aiming to detect chronic health issues and address service deficits. The providers will work closely with primary care doctors, family members, and other healthcare professionals to guarantee seamless care and maintain precise documentation.
Responsibilities
- Conduct thorough health assessments in the home environment for older adults and at-risk populations.
- Identify chronic health conditions and work to close care gaps.
- Coordinate with primary care physicians and other healthcare personnel.
- Document care plans and assessments accurately.
- Engage with family members regarding patient care and treatment options.
Qualifications
- Master’s degree in nursing or a related field.
- Current Nurse Practitioner licensure with prescriptive authority.
- Board certification in a relevant medical specialty.
- Current BLS, ACLS, or CPR certification.
