We are seeking an experienced Outpatient Nurse Practitioner or Physician Assistant to join our team in Delmar, New York. Responsibilities
- Deliver comprehensive patient evaluations and care management.
- Collaborate with physicians and healthcare teams to optimize patient outcomes.
- Utilize advanced practice skills to diagnose and treat various conditions.
- Participate in the development and implementation of patient care plans.
- Educate patients and their families on health management and preventive care.
Qualifications
- Must hold current certification as a Physician Assistant or Nurse Practitioner.
- Possess a valid New York state license to practice in the required role.
- Experience in an outpatient care environment is preferred.
