Job description
The practitioner will evaluate patients within Assisted Living Facilities (ALF) and Skilled Nursing Facilities (SNF), delivering palliative care and pain management solutions. Key duties involve establishing diagnoses, prescribing medications, and accurately recording treatment plans in patient records. Applicants must hold a valid Nurse Practitioner or Physician Assistant license in California, along with an active DEA license for this role.
