Job Summary
The Patient Navigator plays a crucial role in facilitating access to behavioral health and dental services for patients. This position involves screening individuals, organizing appointments, and ensuring accurate record-keeping within the system. Additionally, the Patient Navigator engages in outreach efforts and advocates for patients to connect them with essential health and social resources.
Responsibilities
- Screen and schedule patients for behavioral health and dental appointments.
- Maintain precise records in the system for tracking patient interactions.
- Conduct outreach initiatives to educate and assist individuals seeking health services.
- Provide advocacy for patients to help them access necessary health and social support services.
Qualifications
- A High School Diploma or GED is required.
- Possession of a valid driver’s license.
- Proficiency in Microsoft Office applications.
- Strong interpersonal skills for effectively interacting with a diverse clientele.
