**Job Title: Patient Navigator**
The Patient Navigator is responsible for enhancing patient access to healthcare services, which includes scheduling appointments, offering interpretation services, and aiding in the completion of necessary forms. This role also involves collaboration with medical personnel to ensure effective care delivery while guiding patients in finding community resources and addressing their transportation needs. Applicants should possess a minimum of one year of post-secondary education in healthcare or social services and have at least two years of pertinent experience. Proficiency in Burmese, Hakha Chin, and English is essential, along with strong skills in Microsoft Office and electronic medical record (EMR) systems.
