**Patient Navigator**
The Patient Navigator plays a vital role in coordinating care and supporting clients during the intake process to ensure they access the relevant programs. This position involves keeping precise client records, tracking outcomes, and fostering communication among clients, healthcare providers, and the community. Applicants should have a Bachelor’s degree or comparable experience in the Human Services sector and demonstrate moderate to advanced proficiency in computer skills. The role necessitates working with individuals facing significant emotional challenges or mental illness and requires the completion of Case Management Training within six months of employment.
