Job Summary
The Behavioral Health Social Worker is responsible for performing patient evaluations, delivering counseling services, and connecting patients and families with necessary resources. This role involves close collaboration with healthcare professionals to aid in informed treatment decisions and ensure comprehensive case management across diverse care environments.
Responsibilities
- Conduct thorough patient assessments to identify needs and concerns.
- Provide individual and group counseling to patients and families.
- Facilitate connections to community resources and support services.
- Collaborate with medical staff to support effective treatment planning.
- Deliver casework services in various healthcare settings.
Qualifications
- A Master’s degree in Social Work, Counseling, or a related field.
- Minimum of one year of clinical experience in a relevant setting.
- Active state license as a Clinical Social Worker (CSW), Licensed Social Worker (LSW), or Licensed Professional Counselor (LPC).
- Basic Life Support (BLS) certification is mandatory.
