Job Summary
The Patient Transition Liaison is responsible for assessing patient requirements and facilitating smooth transitions among home health, palliative, and hospice care divisions. This role emphasizes the establishment of robust relationships with clinical teams and referral partners, ensuring the accurate flow of information and achieving admission targets.
Responsibilities
- Evaluate patient needs to facilitate effective transitions between various care divisions.
- Collaborate with clinical teams and referral partners to support patient care.
- Ensure the accurate exchange of information among all stakeholders.
- Meet and maintain admission quotas consistently.
Qualifications
- Minimum of two years’ experience in healthcare or marketing.
- Bachelor’s degree is preferred.
- Strong interpersonal and organizational skills are essential.
- Excellent communication abilities are required.
- Must possess a valid driver’s license.
