Job Summary
The Physical Therapist is responsible for assessing patients and developing individualized treatment strategies aimed at enhancing mobility, strength, and overall functional independence. This role necessitates effective collaboration with multidisciplinary teams and nursing personnel to maintain a seamless continuum of care while monitoring the progress of patients.
Responsibilities
- Conduct comprehensive evaluations of patients to assess their physical therapy needs.
- Design and implement tailored treatment plans focused on improving mobility and strength.
- Work collaboratively with other healthcare professionals to ensure integrated patient care.
- Monitor and document patient progress regularly to adjust treatment strategies as necessary.
- Engage in ongoing education and development to enhance skills, particularly in geriatric rehabilitation.
Qualifications
- Must possess a valid Physical Therapist license or be license eligible in the state of practice.
- Graduate degree from an accredited Physical Therapist program is mandatory.
- A strong interest in geriatric rehabilitation is preferred.
