Job Summary
The Physical Therapist is responsible for delivering evaluative, consultative, and therapeutic interventions to patients in a home setting with the goal of restoring functionality and alleviating discomfort. This role involves close collaboration with an interdisciplinary team and effective management of individualized patient care plans.
Responsibilities
- Conduct comprehensive evaluations and implement treatment interventions in patients’ homes.
- Document clinical findings and maintain accurate patient records.
- Work collaboratively with healthcare professionals to develop and evaluate patient care strategies.
- Adjust treatment plans based on patient progress and needs.
- Educate patients and their families on rehabilitation exercises and lifestyle adjustments.
Qualifications
- Must possess a valid professional physical therapy license in the designated state of practice.
- Graduation from an accredited physical therapy educational program is required.
- Experience in home health settings is advantageous, although not mandatory.
