Job Summary
This role involves providing physical therapy services in the comfort of patients’ homes under the guidance of a licensed Physical Therapist. The focus is on enhancing patients’ mobility, strength, balance, and overall functional independence.
Responsibilities
- Deliver physical therapy sessions in patients’ residences, ensuring alignment with prescribed therapeutic plans.
- Support and instruct patients and their caregivers in exercises and safe movement techniques.
- Record therapy sessions and patient progress in WellSky/Kinnser accurately.
- Communicate updates and any concerns regarding patient status to the healthcare team promptly.
Qualifications
- Completion of a Physical Therapy Assistant program from an accredited institution.
- Possession of an active Physical Therapy Assistant license in California.
- Certification in CPR and BLS.
- Access to reliable transportation with a readiness to travel to various patient locations.
- Prior experience in home healthcare settings and familiarity with Kinnser/WellSky systems are advantageous.
