Job Summary
The physician will deliver exceptional end-of-life care to patients and their families utilizing both in-person and telehealth consultations. This role is integral in fostering collaboration with clinical teams and primary care physicians to create and execute tailored care plans that reflect the unique needs and desires of each patient.
Responsibilities
- Provide comprehensive end-of-life care to patients and their families through face-to-face and virtual visits.
- Collaborate with multidisciplinary clinical teams and primary care providers.
- Develop and implement individualized care plans based on patient preferences and requirements.
- Advocate for patients’ goals and ensure effective communication throughout the care process.
Qualifications
- Must possess a Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree.
- Demonstrated strong commitment to hospice and palliative care.
- Excellent communication skills and the ability to advocate for patients.
