Job Summary
The role involves delivering essential primary healthcare services to individuals residing in long-term care settings. This includes conducting physical examinations and diagnosing medical conditions while working closely with a multidisciplinary team to create tailored care strategies. The provider will also be responsible for educating both patients and their families about various health-related matters.
Responsibilities
- Provide primary care services to residents in long-term care facilities.
- Perform physical assessments and diagnose health issues.
- Collaborate with interdisciplinary teams to formulate individualized care plans.
- Offer education and support to patients and their families regarding health conditions.
Qualifications
- Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license in Ohio.
- A minimum of 6 months of experience in a long-term care setting or skilled nursing facility.
