Job Summary
The primary function of this position is to offer essential primary care services to individuals residing in skilled nursing facilities. This includes conducting thorough physical examinations, diagnosing various health issues, and collaborating with healthcare teams to formulate tailored care strategies.
Responsibilities
- Provide primary care services to residents in skilled nursing environments.
- Conduct physical assessments and diagnose health conditions.
- Work alongside interdisciplinary teams to create personalized care plans.
- Educate families regarding treatment objectives and care plans.
Qualifications
- Active and unrestricted license as a Nurse Practitioner, Physician Assistant, or Physician in the state of Ohio.
- A minimum of 6 months of experience in long-term care or in a skilled nursing facility setting.
