Job Title: Preventive Primary Care Provider for Adults 65+
Responsibilities include delivering preventive primary care services to adults aged 65 and older, with an emphasis on managing chronic diseases. Collaborate effectively within an integrated care team that comprises nurse practitioners, physician assistants, and social workers. Candidates must be Board Certified or Board Eligible in Internal Medicine or Family Medicine. Excellent communication abilities and a strong aptitude for working within a value-based care framework are essential.
