Job description
Deliver preventive primary healthcare and manage chronic conditions for individuals aged 65 and up within a value-driven care framework. Work alongside a collaborative care team that includes Nurse Practitioners, Physician Assistants, medical assistants, and social workers to ensure optimal patient health coordination. Applicants should hold Board Certification or be Board Eligible in Internal Medicine or Family Medicine. While experience in geriatrics and value-based care is advantageous, it is not a mandatory requirement.
