Job description
**Job Title:** Home Health Nurse for Medically Fragile Youth
Deliver individual in-home nursing support for medically delicate children and young adults. Conduct patient evaluations, implement treatment plans, and ensure continuous communication with families and healthcare providers. An active Registered Nurse (RN) or Licensed Vocational Nurse (LVN) license and a current CPR certification are required. Experience in pediatric home health is advantageous, though recent graduates are encouraged to submit their applications.
