**Job Title: Recovery Social Work Care Manager**
The Recovery Social Work Care Manager is responsible for coordinating screenings and interventions for individuals with intricate chronic care requirements, serving as a vital link between patient needs and accessible healthcare services. This position involves delivering therapeutic support, advocating for patients in negotiations with insurance and social service organizations, and working in collaboration with an interdisciplinary team to enhance overall health outcomes. Applicants should possess a bachelor’s degree in social work along with a valid Illinois LSW license. The role mandates 2-3 years of clinical experience, expertise in EMR systems, and the capability to perform physical tasks, including visits to homes or hospitals.
