Job description
The Registered Nurse Assessment Coordinator oversees the resident evaluation process, which encompasses the Minimum Data Set and the development of interdisciplinary care plans. This role involves tracking patient status and their advancement toward predetermined goals, while working closely with the healthcare team to guarantee high-quality care throughout the admission and discharge phases. Applicants must have a valid, unrestricted RN license and a minimum of two years of hands-on nursing experience in long-term care. Familiarity with ICD-10 coding and experience with the Resident Assessment Instrument process is essential.
