Job Summary

Responsibilities
- Provide direct, one-on-one nursing care to patients within their homes.
- Develop and implement individualized care plans based on patient needs.

- Maintain accurate and up-to-date documentation in line with regulations.
Qualifications
- Must possess a valid RN license in Colorado.
- Current CPR certification is required.
- A valid driver’s license and dependable transportation are necessary for home visits.
